 By using Plastictox, the user consents to the following:

• You can form a binding contract with Arrow Lab Solutions
• You are not a person barred from receiving Plastictox under the laws of the jurisdiction from which you use Plastictox
• You are at least 18 years old
• Any sample you provide to Arrow Lab Solutions is either your own, or the sample of a minor for whom you are a parent or legal guardian, or the sample of a person for whom you are a legally authorized representative
• The sample you provide is collected as described in the product insert
• You are not an insurance company or an employer

Once you obtain your lab report, the knowledge is irrevocable. You should not assume that any information provided to you, whether now or as our research advances, will be welcome or positive. As research advances, you may need to obtain further services from Arrow Lab Solutions, your physician, a counsellor, or other healthcare provider. We encourage you to talk to a physician to help you understand your results and testing options.

Arrow Lab Solutions is not responsible for outside services, counselling, or recommendations that are sought out as interpretations of reports generated by Plastictox or any Arrow Lab Solutions product.

• Once payment is received, please allow 2-3 weeks for test kit dispatch
• Test results from SV Biotech B.V. will be sent to the address provided between  2-4 weeks of receipt
• Plastictox UK Ltd is the sole UK Distributor on behalf of Plastictox.com (US)
